| Định vị dịch vụ Elite Care cung cấp hệ thống chăm sóc tại nhà được vận hành theo đánh giá nhu cầu, kế hoạch cá nhân hóa, bố trí Chăm sóc viên phù hợp, điều phối, giám sát chất lượng và báo cáo. Dịch vụ chăm sóc không thay thế bác sĩ, điều dưỡng hoặc cơ sở khám bệnh, chữa bệnh trong các hoạt động thuộc phạm vi chuyên môn y tế. |
1. Mục đích và phạm vi
Chính sách này mô tả nguyên tắc cung cấp dịch vụ chăm sóc tại nhà Elite Care, quyền và trách nhiệm cơ bản của các bên, quy trình trước - trong - sau khi chăm sóc, giới hạn chuyên môn, an toàn, điều phối, báo cáo và cơ chế hỗ trợ. Chính sách áp dụng cùng Hợp đồng/Báo giá/Kế hoạch chăm sóc và các chính sách website có liên quan.

2. Đối tượng và nhóm nhu cầu phục vụ
Elite Care tập trung hỗ trợ người cần chăm sóc tại nhà, bao gồm các nhóm thường gặp như người sau đột quỵ, sau phẫu thuật, người cao tuổi suy yếu, người bệnh cần chăm sóc cuối đời và trường hợp khác phù hợp với phạm vi dịch vụ/năng lực đáp ứng.
Việc nhận ca không dựa chỉ vào tên bệnh. Elite Care xem xét tình trạng thực tế, mức độ phụ thuộc, môi trường tại nhà, mục tiêu chăm sóc, phạm vi chuyên môn, yêu cầu an toàn và khả năng bố trí nhân sự.
3. Nguyên tắc cung cấp dịch vụ
Đúng người: bố trí Chăm sóc viên phù hợp với tình trạng, mức độ hỗ trợ và yêu cầu của ca.
Đúng kế hoạch: chăm sóc theo mục tiêu, nội dung và mức hỗ trợ đã được xác nhận.
Đúng quy trình: tuân thủ quy trình Elite Care, an toàn người bệnh và kiểm soát nhiễm khuẩn trong phạm vi áp dụng.
Đúng thời gian: điều phối lịch theo thỏa thuận và chủ động phối hợp khi có nguy cơ gián đoạn.
Tôn trọng và bảo mật: tôn trọng nhân phẩm, quyền riêng tư, phong tục, tài sản và dữ liệu khách hàng.
Báo cáo minh bạch: ghi nhận hoạt động chăm sóc và dấu hiệu cần lưu ý theo quy trình.
Cải tiến liên tục: tiếp nhận phản hồi, giám sát chất lượng và điều chỉnh kế hoạch khi tình trạng/nhu cầu thay đổi.
4. Quy trình triển khai dịch vụ

5. Nội dung chăm sóc theo mô hình Elite Care
5.1. Chăm sóc thể lý và sinh hoạt hằng ngày
Hỗ trợ vệ sinh cá nhân, ăn uống, nghỉ ngơi, bài tiết và sinh hoạt hằng ngày theo kế hoạch.
Theo dõi các chỉ số sức khỏe hoặc dấu hiệu cần lưu ý theo kế hoạch/khả năng được phân công.
Hỗ trợ sử dụng thuốc theo hướng dẫn đã có của bác sĩ/người có chuyên môn y tế hành nghề khám bệnh chữa bệnh; không tự ý kê đơn, đổi liều, ngưng thuốc hoặc thay phác đồ khi chưa có sự chỉ định của bác sĩ/người có chuyên môn y tế phụ trách bệnh án, sức khỏe của người được chăm sóc.
Hỗ trợ phòng ngừa nguy cơ do bất động như loét tì đè, té ngã, sặc và duy trì môi trường an toàn trong phạm vi chăm sóc.
5.2. Hỗ trợ vận động và khả năng tự lập
Thay đổi tư thế; vận động thụ động/có trợ giúp/chủ động theo kế hoạch và năng lực đã được xác nhận.
Hỗ trợ ngồi, đứng, thăng bằng, đi lại với dụng cụ hỗ trợ khi phù hợp.
Khuyến khích thực hiện các hoạt động sinh hoạt hằng ngày nhằm duy trì/tăng khả năng tự lập.
Không tự nhận hoạt động chăm sóc là vật lý trị liệu/phục hồi chức năng chuyên môn nếu không thuộc phạm vi được phép hoạt động kinh doanh.
5.3. Chăm sóc tinh thần và đồng hành
Trò chuyện, lắng nghe, hỗ trợ duy trì giao tiếp và thói quen phù hợp.
Khuyến khích hoạt động tinh thần/giải trí phù hợp với thể trạng, sở thích và không gian sống.
Ghi nhận thay đổi về tâm trạng, hành vi hoặc giao tiếp để báo cáo gia đình/đầu mối phù hợp; không tự chẩn đoán rối loạn tâm thần.
6. Ranh giới chuyên môn và các việc không thuộc dịch vụ
| Không thay thế khám bệnh, chữa bệnh Chăm sóc viên Elite Care không mặc nhiên là bác sĩ/điều dưỡng/KTV phục hồi chức năng. Chỉ những hoạt động phù hợp phạm vi dịch vụ, năng lực đã đánh giá và quy định pháp luật mới được thực hiện. Khi nhu cầu vượt phạm vi, Elite Care hướng dẫn/chuyển đầu mối phù hợp. |
Không tự chẩn đoán bệnh, kê đơn, thay đổi thuốc, phác đồ hoặc cam kết kết quả điều trị/thời gian hồi phục.
Không thực hiện tiêm, truyền, thủ thuật hoặc kỹ thuật y tế ngoài phạm vi được phép/chưa được Elite Care xác nhận.
Không nhận công việc nhà tổng quát ngoài phạm vi chăm sóc chỉ để đáp ứng yêu cầu phát sinh.
Không vay/mượn tiền, tài sản; không nhận lợi ích ngoài thỏa thuận; không lôi kéo khách hàng sử dụng dịch vụ ngoài hệ thống Elite Care.
7. Bố trí và thay thế Chăm sóc viên
Elite Care lựa chọn Chăm sóc viên dựa trên nhu cầu, mức độ chăm sóc, năng lực/kinh nghiệm, giới tính hoặc yêu cầu hợp lý của gia đình, vị trí địa lý và lịch nhân sự. Elite Care nỗ lực duy trì tính liên tục nhưng không cam kết một Chăm sóc viên cố định tuyệt đối nếu không được ghi rõ trong hợp đồng.
Khách hàng có thể phản hồi nếu Chăm sóc viên không phù hợp; Elite Care đánh giá nguyên nhân và phương án điều chỉnh/thay thế.
Thay thế khẩn cấp phụ thuộc độ phức tạp của ca và nguồn nhân sự tại thời điểm phát sinh; Elite Care không hứa SLA tuyệt đối nếu chưa được quy định riêng.
Elite Care có thể thay Chăm sóc viên vì nghỉ bệnh, an toàn, vi phạm quy trình, điều phối hoặc lý do hợp lý khác; khách hàng được thông báo trong khả năng sớm nhất.
8. Trách nhiệm của khách hàng/gia đình
Cung cấp thông tin trung thực, đầy đủ về tình trạng sức khỏe, thuốc, dị ứng, lịch khám/chỉ định, nguy cơ và thay đổi quan trọng mà gia đình biết.
Cung cấp môi trường làm việc an toàn, điều kiện vệ sinh cơ bản, nước/điện/dụng cụ cần thiết theo phương án chăm sóc; thông báo nguy cơ trong nhà (thú nuôi, camera, người có hành vi bạo lực, hút thuốc, v.v.).
Không yêu cầu Chăm sóc viên thực hiện công việc ngoài phạm vi của dịch vụ chăm sóc sức khỏe hoặc hành vi trái pháp luật/trái an toàn.
Phối hợp đưa người bệnh đến cơ sở y tế khi có dấu hiệu cần khám/cấp cứu; không trì hoãn chỉ vì đang sử dụng dịch vụ chăm sóc tại nhà.
Tôn trọng Chăm sóc viên; không quấy rối, bạo lực, xúc phạm hoặc yêu cầu giao dịch tài chính cá nhân.
Thanh toán và thông báo hủy/đổi theo chính sách/hợp đồng đã xác nhận.
9. An toàn, sự cố và tình huống khẩn cấp
An toàn người được chăm sóc là ưu tiên. Chăm sóc viên theo dõi dấu hiệu bất thường trong phạm vi được phân công, thực hiện hỗ trợ ban đầu phù hợp năng lực và báo cáo Elite Care/người liên hệ của khách hàng.
Khi có nguy cơ đe dọa tính mạng hoặc tình trạng khẩn cấp, liên hệ 115/cơ sở y tế phù hợp; dịch vụ Elite Care không phải dịch vụ cấp cứu.
Sự cố như té ngã, sặc, chấn thương, mất tài sản, cháy/nổ, hành vi bạo lực hoặc sự cố bảo mật phải được ghi nhận và báo cáo theo quy trình.
Elite Care có thể tạm ngưng hoạt động chăm sóc nếu môi trường có nguy cơ nghiêm trọng cho người bệnh hoặc Chăm sóc viên cho đến khi có phương án an toàn.
10. Báo cáo, bàn giao và giám sát chất lượng
Chăm sóc viên hoàn thành nhật ký/báo cáo theo quy định và bàn giao các nội dung quan trọng khi kết thúc ca/chuyển ca.
Gia đình được cập nhật thông tin chăm sóc theo cơ chế báo cáo của Elite Care và mức quyền truy cập phù hợp.
Kiểm soát chất lượng có thể rà soát việc tuân thủ kế hoạch, phản hồi khách hàng, dấu hiệu bất thường và đề xuất điều chỉnh.
Kế hoạch chăm sóc có thể thay đổi khi tình trạng/khả năng/nguyện vọng thay đổi; thay đổi quan trọng về phạm vi/chi phí phải được xác nhận trước.
11. Quyền riêng tư, hình ảnh và tài sản
Elite Care và Chăm sóc viên tôn trọng quyền riêng tư, bảo mật thông tin và tài sản của khách hàng.
Việc chụp/quay hình ảnh để phục vụ chăm sóc/báo cáo chỉ thực hiện theo quy trình, mục đích cần thiết và quyền truy cập được kiểm soát.
Việc sử dụng hình ảnh/câu chuyện cho truyền thông, marketing cần cơ chế đồng ý riêng phù hợp; khách hàng có quyền từ chối mà không ảnh hưởng đến chất lượng dịch vụ.
Gia đình cần thông báo nếu có camera giám sát tại khu vực chăm sóc; việc sử dụng camera phải tuân thủ quyền riêng tư và pháp luật.
12. Giá, thanh toán, hủy/đổi và hoàn phí
Giá và kỳ thanh toán theo Báo giá/Hợp đồng/Xác nhận dịch vụ. Hủy, đổi lịch, tạm ngưng, chấm dứt và hoàn phí thực hiện theo Chính sách Hủy - Đổi lịch - Chấm dứt dịch vụ - Hoàn phí và Chính sách Thanh toán đang có hiệu lực, cùng các điều kiện cụ thể đã được khách hàng xác nhận.
13. Tạm ngưng hoặc chấm dứt dịch vụ bởi Elite Care
Elite Care có thể đề nghị tạm ngưng/chấm dứt hoặc chuyển hướng dịch vụ khi việc tiếp tục không còn an toàn, vượt phạm vi chuyên môn, khách hàng liên tục yêu cầu hành vi trái quy định, có bạo lực/quấy rối, không cung cấp điều kiện chăm sóc tối thiểu hoặc vi phạm nghĩa vụ thanh toán theo thỏa thuận.
Trừ tình huống khẩn cấp/bất khả kháng hoặc pháp luật cho phép xử lý ngay, Elite Care thông báo và phối hợp phương án chuyển tiếp hợp lý, đối soát phần dịch vụ đã sử dụng và phần phí chưa sử dụng theo chính sách/hợp đồng.
14. Người tiêu dùng dễ bị tổn thương
Khi người sử dụng/người được chăm sóc thuộc nhóm người tiêu dùng dễ bị tổn thương, Elite Care áp dụng Chính sách Bảo vệ Người tiêu dùng Dễ bị tổn thương, ưu tiên cơ chế giải thích, hỗ trợ giao tiếp, tiếp nhận yêu cầu và khiếu nại phù hợp với từng trường hợp.
15. Phản hồi và khiếu nại
Khách hàng có thể phản hồi qua Hotline 0931 77 22 80 hoặc Email cskh@elite-healthcare.com.vn. Khiếu nại được xử lý theo Chính sách Tiếp nhận và Giải quyết khiếu nại. Phản ánh về an toàn, tài sản, bảo mật hoặc nghi ngờ lạm dụng được ưu tiên escalation.
16. Hiệu lực và thứ tự áp dụng
Chính sách có hiệu lực từ 28/09/2026. Với một ca cụ thể, Hợp đồng/Báo giá/Kế hoạch chăm sóc được ưu tiên về nội dung chi tiết đã thỏa thuận; các chính sách website bổ trợ trong phạm vi tương ứng. Không nội dung nào được hiểu là loại trừ quyền bắt buộc của người tiêu dùng hoặc trách nhiệm mà Elite Care phải chịu theo pháp luật.

HOME CARE SERVICE PROVISION POLICY
| Service Positioning Elite Care provides a home care system operated through needs assessment, personalized care planning, assignment of suitable Caregivers, coordination, quality monitoring, and reporting. Care services do not replace doctors, nurses, or medical examination and treatment facilities in activities falling within the scope of professional medical practice. |
1. Purpose and Scope
This Policy describes the principles governing Elite Care home care services, the basic rights and responsibilities of the parties, the process before, during, and after care, professional boundaries, safety, coordination, reporting, and support mechanisms. This Policy applies together with the Service Agreement/Quotation/Care Plan and relevant website policies.

2. Service Recipients and Categories of Care Needs
Elite Care focuses on supporting persons who require home care, including common groups such as persons recovering from stroke or surgery, frail elderly persons, patients requiring end-of-life care, and other cases appropriate to the scope of services and available capacity.
Acceptance of a care case is not based solely on the name of a disease or condition. Elite Care considers the actual condition, level of dependence, home environment, care objectives, professional scope, safety requirements, and staff availability.
3. Service Provision Principles
Right Person: assign a Caregiver appropriate to the condition, level of support, and requirements of the care case.
Right Plan: provide care in accordance with the confirmed objectives, scope, and level of support.
Right Process: comply with Elite Care procedures, patient safety requirements, and infection control measures within the applicable scope.
Right Timing: coordinate schedules as agreed and proactively make arrangements where there is a risk of service disruption.
Respect and Confidentiality: respect the dignity, privacy, customs, property, and data of Customers.
Transparent Reporting: record care activities and signs requiring attention in accordance with procedures.
Continuous Improvement: receive feedback, monitor quality, and adjust the plan when the condition or needs change.
4. Service Implementation Process

5. Care Activities under the Elite Care Model
5.1. Physical Care and Activities of Daily Living
Assist with personal hygiene, eating and drinking, rest, toileting, and activities of daily living in accordance with the plan.
Monitor health indicators or signs requiring attention in accordance with the plan and assigned capabilities.
Assist with medication use in accordance with existing instructions from a doctor or healthcare professional qualified to practice medical examination and treatment; do not independently prescribe medication, change dosage, discontinue medication, or alter a treatment regimen without instructions from the doctor or healthcare professional responsible for the Care Recipient’s medical record and health.
Assist in preventing risks associated with immobility, such as pressure ulcers, falls, and choking, and maintain a safe environment within the scope of care.
5.2. Mobility Support and Independence
Assist with repositioning and passive/assisted/active movement in accordance with the plan and confirmed capabilities.
Assist with sitting, standing, balance, and mobility using assistive devices where appropriate.
Encourage activities of daily living to maintain or improve independence.
Care activities must not be represented as professional physical therapy/rehabilitation where such activities fall outside the permitted scope of business operations.
5.3. Emotional Care and Companionship
Engage in conversation, listen, and support appropriate communication and routines.
Encourage mental and recreational activities appropriate to the physical condition, preferences, and living environment.
Record changes in mood, behavior, or communication for reporting to the family or appropriate contact person; do not independently diagnose mental disorders.
6. Professional Boundaries and Activities Outside the Scope of Services
| Does Not Replace Medical Examination and Treatment Elite Care Caregivers are not automatically doctors, nurses, or rehabilitation technicians. Only activities consistent with the service scope, assessed capabilities, and applicable law may be performed. Where a need exceeds that scope, Elite Care will provide guidance or refer the matter to an appropriate provider/contact point. |
Do not independently diagnose disease, prescribe medication, change medication or treatment regimens, or guarantee treatment outcomes/recovery periods.
Do not perform injections, infusions, procedures, or medical techniques outside the permitted scope or not confirmed by Elite Care.
Do not undertake general household work outside the scope of care merely to satisfy ad hoc requests.
Do not borrow or lend money or property; do not accept benefits outside the agreed arrangements; and do not solicit Customers to use services outside the Elite Care system.
7. Assignment and Replacement of Caregivers
Elite Care selects Caregivers based on needs, level of care, capability/experience, gender or reasonable family requirements, geographic location, and staff schedules. Elite Care endeavors to maintain continuity but does not guarantee an absolutely fixed Caregiver unless expressly stated in the agreement.
Customers may provide feedback if a Caregiver is not suitable; Elite Care will assess the reason and an appropriate adjustment/replacement option.
Emergency replacement depends on the complexity of the care case and staff availability at the relevant time; Elite Care does not guarantee an absolute SLA unless separately specified.
Elite Care may replace a Caregiver due to illness, safety concerns, procedural violations, coordination needs, or other reasonable grounds; the Customer will be notified as soon as reasonably possible.
8. Responsibilities of the Customer/Family
Provide truthful and complete information known to the family regarding health condition, medications, allergies, examination schedules/medical instructions, risks, and material changes.
Provide a safe working environment, basic sanitary conditions, and the water/electricity/equipment necessary under the care arrangement; disclose household risks (pets, cameras, persons exhibiting violent behavior, smoking, etc.).
Do not require a Caregiver to perform work outside the scope of healthcare services or any unlawful or unsafe act.
Cooperate in taking the patient to an appropriate medical facility when signs indicate a need for medical examination or emergency care; do not delay solely because home care services are being used.
Respect Caregivers; do not harass, use violence against, insult, or request personal financial transactions from them.
Make payments and provide cancellation/change notices in accordance with the confirmed policy/agreement.
9. Safety, Incidents, and Emergencies
The safety of the Care Recipient is a priority. Caregivers monitor abnormal signs within their assigned scope, provide initial support appropriate to their capabilities, and report to Elite Care/the Customer’s contact person.
Where there is a life-threatening risk or emergency, contact 115/an appropriate medical facility; Elite Care services are not emergency services.
Incidents such as falls, choking, injury, loss of property, fire/explosion, violent conduct, or security incidents must be recorded and reported in accordance with procedures.
Elite Care may suspend care activities if the environment poses a serious risk to the patient or Caregiver until a safe arrangement is in place.
10. Reporting, Handover, and Quality Monitoring
Caregivers complete logs/reports as required and hand over important information at the end of a shift or when changing shifts.
The family is updated on care information in accordance with Elite Care’s reporting mechanism and the appropriate level of access rights.
Quality Control may review compliance with the plan, Customer feedback, abnormal signs, and proposed adjustments.
The Care Plan may be adjusted when the condition, capabilities, or preferences change; material changes to scope or cost must be confirmed in advance.
11. Privacy, Images, and Property
Elite Care and Caregivers respect the Customer’s privacy, confidentiality of information, and property.
Photographs/recordings for care or reporting purposes may only be made in accordance with procedures, for necessary purposes, and subject to controlled access rights.
Use of images/stories for communications or marketing requires an appropriate separate consent mechanism; the Customer has the right to refuse without affecting service quality.
The family must disclose any surveillance cameras in the care area; use of cameras must comply with privacy rights and applicable law.
12. Price, Payment, Cancellation/Changes, and Refunds
Prices and payment periods are governed by the Quotation/Agreement/Service Confirmation. Cancellation, rescheduling, suspension, termination, and refunds are handled in accordance with the then-current Cancellation - Rescheduling - Service Termination - Refund Policy and Payment Policy, together with the specific conditions confirmed by the Customer.
13. Suspension or Termination of Services by Elite Care
Elite Care may propose suspension/termination or redirection of services where continuation is no longer safe, exceeds the professional scope, the Customer repeatedly requests non-compliant conduct, violence/harassment occurs, minimum care conditions are not provided, or agreed payment obligations are breached.
Except in an emergency/force majeure event or where the law permits immediate action, Elite Care will provide notice and coordinate a reasonable transition arrangement, reconcile services already used, and handle unused fees in accordance with the applicable policy/agreement.
14. Vulnerable Consumers
Where a User/Care Recipient is a vulnerable consumer, Elite Care applies its Vulnerable Consumer Protection Policy, prioritizing appropriate explanation, communication support, receipt of requests, and complaint handling on a case-by-case basis.
15. Feedback and Complaints
Customers may provide feedback via Hotline 0931 77 22 80 or Email cskh@elite-healthcare.com.vn. Complaints are handled in accordance with the Complaint Receipt and Resolution Policy. Reports concerning safety, property, security, or suspected abuse are prioritized for escalation.
16. Effectiveness and Order of Application
This Policy takes effect on September 28, 2026. For a specific care case, the Service Agreement/Quotation/Care Plan prevails with respect to the detailed terms agreed for that case; website policies supplement it within their respective scopes. Nothing herein shall be construed as excluding any mandatory consumer right or any liability that Elite Care is required to bear under applicable law.
